What you're asking the court to find
This part is a proposal, not a finding. A judge has to sign the form before it means anything, and the court can turn down a deviation and send you back to redo it. If you and the other parent settled this in mediation, that agreement is what you are asking the judge to approve.

The Court sets the final figure and may deviate from the guidelines on the record. It does not cover the OKDHS Child Care Subsidy worksheet, and the guideline schedule stops at $15,000 of combined monthly income — above that the Court has discretion.
